Five Options to Better Serve You:
| Option #1: |
New Patients - cash, check or credit
card (MasterCard, Visa, Discover) is required at all first visits. |
| Option #2: |
For fees under $200, payment by cash,
check or credit card is required at the time of service. |
| Option #3: |
Payment can be divided over the time
of treatment. If treatment takes two appointments, two payments
can be made. Three appointments, three visits |
| Option #4: |
One half paid at the start of treatment
with the balance payable in three or four equal payments over a
three to four month period. |
| Option #5: |
Payment may be spread over a three
to ten month period before treatment is started. This option can
be customized by our ofice manager and we will generate a payment
schedule for you. |
